Title: FIXED: SteamVR Error 475 (VRServer failed to start / IPC timeout)
- Loose or damaged cable – The multi-cable connecting the headset to the link box or directly to the PC is partially disconnected.
- USB port incompatibility – Using USB 3.0 ports on certain motherboards (especially AMD X570/B550 chipsets) is a known culprit.
- DisplayPort/HDMI handshake failure – The GPU and headset fail to negotiate a signal.
- Corrupted USB drivers or SteamVR USB devices – Windows accumulates “ghost” devices.
- Firmware mismatch – The headset firmware is outdated or corrupted after a failed update.
- Direct Display Mode disabled – SteamVR isn’t taking exclusive control of the headset’s display.
- Power management – Windows turns off the USB root hub to save power.
