How to Install a Memory Card Save File for Naruto Shippūden: Ultimate Ninja 5 on PCSX2 Naruto Shippūden: Ultimate Ninja 5

If your downloaded/converted file is named custom.ps2, rename it to Mcd001.ps2 (this makes it the primary memory card in Slot 1).
Paste it into the memcards folder, overwriting if prompted.

Using PCSX2 Memory Card Manager (if needed)

Using myMC, open your Mcd001.ps2 file.

Click Import (usually an icon with an arrow pointing into a folder).

Select the downloaded save file (ensure it is extracted from the .zip/.rar).

The save will now appear inside your memory card list.

Save changes in myMC, close it, and launch your game on PCSX2.
