- Ironworks—on Forgemother’s platform (post-arm disable)
- Final Sanctuary—requires unlocking both route gates
- Die at least 12 times, including specific deaths: spike trap (Ch1), drowning (Ch3), False Prophet (Ch4), and 9 other random deaths.
- Collect all 5 Broken Hourglass pieces (locations summarized below).
- Do not use the Golden Hourglass in Chapter 4.
